Cancellation form

Dear medihelp. On Thursday I requested a cancellation form via email. Your automated email was kind enough to tell me that it is been taking care of. Funny enough I haven't received the form yet. So where is it? I have made it clear on the email that I will be closing all my accounts incl my bank accounts by mid Feb. I have done my part. You just have to do yours now.
