Unlawful cancellation of medical aid plan

Kind please advised that on the 2nd of September 2013, I sent through a request to Bankmed for a beneficiary resignation.
The request was processed as an outright cancellation of the medical aid in error, instead of the beneficiary being taken off and leaving the cover in force.
3 months passed from the time this request was submitted without notification from Bankmed of the cancellation and me being unaware of this error, I then I paid a visit to my GP and they advised me that my medical aid is inactive and to my knowledge the cover should have still be in force as I still work for Absa and I have not changed medical aid companies.
Please note that Bankmed now does not want to take accountability or responsibility for the incorrect processing of my request, instead they have logged a 3 month arrears premium collection, despite the fact that they made an error on their side. Note that for 3 months,
According to my payslip, I have calculated R 4260 that will be taken as a result of these arrears.

Please help me in anyway that you can.
