Not authorising prevention treatment

I went to my doctor and was advised that I am required to for a Colonoscopy as my father died from Colon cancer. I was advised that I am required to go for these scans every year as pollops were found in my colon previously. I called discovery to see if they would cover this treatment. I was advised that I would need to get a authorisation form filled out by my Keycare Doctor. This has been done several times and have been told that the forms have been incorrectly filled out. My doctor and I have no idea what else is needed on this form. At this moment I feel like I am being pushed from Pillar to post with my health not being a Priority. I have had several communications with the relevant department with no luck.

Please could someone assist me here
