Cover Failure incompetent service

I had a procedure done at the Cape Eye Hospital on June 6th 2013. All the procedure codes were given to me by the surgeon and anesthetist and approved by Discovery when I called then, with the guarantee of no shortfall. A few weeks after the procedure I received an email statement from Discovery with a breakdown off the costs and the balance of nil.
About 3 weeks ago I received a statement from the Cape Eye Hospital stating the I owed about R2500 which Discovery had reversed. I called the Discovery call center and spoke to about 10 different consultants and no one could give me clarity or answers? I received an email from the head of their service department asking if my issue had been sorted out. When I replied that it hadn't, I received no feedback? After about 2 weeks of phone calls to Discovery, I was told that one of the procedures was not covered for.
I called my anesthetist's office and they told me that Discovery had decided to take this procedure's cover off their list in August. I had my procedure done in June!? My anesthetist gave me a motivational letter to send to Discovery. I sent it Friday 27/09/2103 and was told I would have feedback within 48-72hrs. Nothing?
