Changing contract and terms without customer OK

As I stopped working in July I have requested to stay a member of momentum on the 26/6/13. I completed all the forms and they requested additional information by email which was sent. They stopped the contract for the company and changed me to a new contract with new bank details. They conveniently picked only items they like but did not change to the plan I requested. No communcition was made from them to tell me they demand changes. Further they claim they could not get hold of me to discuss but they have sent 10 emails to which I all responded so I thought everything is OK. Big was my shock when they collected the debit order for the wrong amount. After many emails and a phone call they eventually came out with the fact they created a new contract which differ from the application form. I requests immediate refund and if they cannot act as per the application form. This is blatant unethically behaviour.
