Disappointing service

I approached Momentum Health as I wanted to apply for a medical aid for my mum. I began the process, on 29/08/2013, when I sent the application forms. From 30/08/2013 to 26/10/2013 I had been sending additional information, as & when Momentum Health requested me to. One of the requests was for an affidavit, from my mum. To ensure that the affidavit contained the required information, I called the call center for guidance. As per the consultant's advise the affidavit was done & sent. After mailing the affidavit I received another e-mail stating that the application still could not be processed. I called the call center & another consultant said that another affidavit was needed as the first one was incorrect. On 21/10/2013 I cancelled the application, stating on my e-mail the reasons for cancellation.
On 04.12.2013 I called Momentum Health, again, to request an appointment with a consultant. I was told that I would be contacted by 09/12/2013. I was not contacted so I called the call center on 11/12/2013. I did not receive any help. To date I have not been contacted to set up an appointment.
It appears that the customer's request & query is not taken seriously & not followed up.
