No accounts sent, then threatening phone calls!

Our 4 year old daughter requires admissions every 3 weeks for infusions, meaning a new hospital account is generated every 3 weeks.
In January this year I suddenly received a threatening phone call demanding payment for accounts >5 months old. I had never received a single account from Life Healthcare, & I was not informed of any outstanding accounts despite being at the hospital every month, & being in contact with the accounts department at Wilgeheuwel, requesting them to let me know of any problems. I also confirmed our postal & email addresses at every admission.
There followed in February & March a long trail of emails back & forth, including with head office about the accounts & how to prevent issues in the future. Since then I've been in regular contact with the patient services manager & have notified them of problems I noted on our medical aid statements (the main problem has always been incorrect coding on accounts, resulting in non-payment) but I have not received any account at all & have not been told of any problems. BUT Today I received another threatening phone call regarding an outstanding account from January this year.
I am desperate to sort this issue out!!
