Incorrect processing

A claim for services done in April 2013 send in for reprocessing was (again) declined. Under the chronic claims there are certain items that Discovery Health pays for. I received a letter with the different codes Discovery says to pay but still declines.

In the meantime an old claim was reprocessed and changed with a negative credit. Why?
This claim (October 2012) was finalised in February 2013
I am very unhappy with the wrongly reprocessing of the claim from October 2012.

Further, I cannot understand why Discovery says that they are paying, but they keep on declining.
