Charged THREE times for the same service

We rented a car in December 2012 and were charged as invoiced on 12 January 2013. A further charge was made to our account on 22 January 2013 which we have queried numerous times with numerous promises of investigation and refund. Instead of the promised refund, Now on 16 February 2013 a further charge has been passed - we have now been charged three times for the one car hire. Never mind the bank fees charged due to our account being overdrawn and the negative impact to our credit record.
