Bad food and service consistently

On three occasions I have waiting half an hour to place an order at the restaurant. Despite having sworn never to return I was persuaded but should have gone with previous experience. On arriving the door to the smoking section was open. When I asked the manager if it should be closed his response was "normally" but it was not closed despite there being children in the restaurant and smoke wafting through.
We ordered a green salad and were delivered a greek salad. It was then changed except we were only informed on delivery of the salad that not all the ingredients were available - no excuse/offer of replacement ingredients or change of our order simply "we don't have it"
I ordered the seafood platter - the fish was not properly cooked. The waiter offered to take it back to the kitchen and cook it further - fish! I decided I was better off leaving early. We left without desert.
