Unauthorized Debit order on Bank Account

I have not given any consent to this unknown company to debit my bank account. I would like to receive information on how to have this amount reversed (bank charges included) and to stop any further debit orders from being deducted.

I need help ASAP!! This has happened a few times. I don't KNOW WHO THE COMPANY OR PERSON IS???
