BANK FAILED TO DISCLOSE INFORMATION

I was charged for an unpaid debit order i went to the Parktown branch to find out from which company was the debit order from, as all my debit orders went off and the consultant said he cannot see the company name and cannot tell me the amount of the debit order and the date. so i am stuck with a fee of an unpaid debit order from an unknown company and the bank failed to tell me which company is that and in order to reverse the debit order fee i need to go to the brach where i opened my Standar Bank account.
