You have complete Failed me

I am disappointed with the fact that the you could not deliver what you are paid to do. You have promised me on numerous occasions that the will get back to me and resolve my matter, I have left numerous messages for you to call me and to follow up on my matter but to no avail - You have treated me exactly the same as the company I have a grievance with.
Based on this I request that you refund all the premiums you have debited from my bank account since you have completely failed me.
I no longer wish to be associated with your company and instruct you to cancel the debit order with immediate effect!
