No response since 2nd September!

I visited the bank on 2nd September to apply for a secondary card for my husband. A few days later I was contacted by a Prestige Acc Manager to tell me they had given me the wrong forms to fill in and my husbands signature was required. We could not get to the bank over the weekend so I phoned on Monday to ask for the forms to be emailed to me. I AM STILL WAITING FOR A RESPONSE FROM THE BRANCH, even though the Prestige Call Centre has assured me that the matter would be attended to. I have sent numerous emails to Prestige and to the Prestige Account Manager (whom I asked to be changed at some stage owing to her lack of response). I have been with Standard Bank for 30 years and now have to research and source a better solution.
