Consultant did not follow customer's instructions

My wife opened an account at the North End branch in Port Elizabeth two months ago. When she opened the account she added two debit orders to the account with the strict instructions that a hold to be put on these debit orders until she gives the instruction to release them. We are paying cash at the moment to catch up on some arrears on two accounts as per arrangements with the account holders. Yesterday those debit orders went through without my wife's instructions. my wife went to see the consultant involved and she denies that my wife asked her to put a hold on these debit orders. I have now paid two accounts twice this month due to incompetence From FNB staff. They do not want to reverse my money back into my wife's account and told her that she will have to go and sort it out with the two account holders, which incidentally are not even aware of any debit orders. I want this money reversed back into my wife's account immediately.
