Claim has still not been paid out after months

This has been the worst customer service I have ever received. To only advise me now that the only document you received is from the doctor who performed the operation. I sent you proof from my family doctor that he had sent you the report. He then sent it to me and I forwarded to you.how can you say you never received any other documents when you sent me an email saying that you had to re-assess the claim. Did you do the first assessment without any documents? I am truly shocked that you have sent me from pillar to post and everytime I query the claim it is yet another document you require.
Every Single Document has been sent through on numerous occasions!!!
I demand this to be rectified!!!
