Travel insurance debited without my permission

I purchased two tickets on Emirates with my gold card and was debited with two amounts of R337 for travel insurance which I had not requested as a gold card has free travel insurance. I had to cancel the air tickets and rescheduled and paid through Flight Centre. for these tickets no travel insurance appeared on my account even though the booking was also with Emirates. it thenbecame clear that the bank was the culprit. i lodged a dispute through the banks branch and have a copy of the transmission. I phoned card division who told me to be patient on the first occasion. Then I purchased another ticket on Emirates and again I was debited with the same travel insurance of R337. I again phoned card division and was told that no dispute request had been received and I was put through to travel insurance who confirmed they had made the debits and they would credit my account. I was shocked that after so many years with the bank this is how they operate. Still I have received no credit to my account.
