Approved, no delivery, application deleted

At the start of May 2013 my credit card was approved after many application attempts. When I followed up on the delivery of my credit card as I received a sms that the card was approved. The consultant informed me that my application was deleted from the system and that Michelle from credit division was responsible. The consultant would not give me a surname. The service I received is absolutely poor and a disgrace.
