Incorrectly billed / Payments not allocated

Ongoing for 2 years now. My mother in law has a policy with OnePlan. My husband and I pay the premiums every month. We had a debit order setup with the company and 3 months in a row they debited the incorrect amount from our account without prior consent. Change to EFT - EFT payments made - proof faxed and emailed. Every 3 months the policy was suspended due to "Non Payment". Every time we phone and email and fax the proof of payment through again and the policy restored.
At the end of March they added a dependent to the policy ((the dependent has his own policy) without consent from us. Again this led to the policy being suspended as it was paid for one dependent only.
If the policy is suspended and you in an accident, they refuse to assist!!
