Credit Card without application

A credit card was opened in my name, activated and initiation fees charged.
I've alerted the bank that I have never applied for the credit card.
I've been receiving repeated SMS notification to come collect the card at Market Street Branch.
Repeated telephone calls with the branch and the Credit Card division have failed to resolve the issue.
department assisted by placing a hold on the card, offered to investigate but no feedback has been received.
I went to the branch with the aim of closing the account, they couldn't do it because there is now an outstanding balance on the card (which I'm not willing to pay)
The card is still active, on my name at the Market Street branch.
- Absa has failed to investigate my complaint and provide feedback.
- Absa has failed to provide details of the application and preferably a copy of the application form.
- Absa has failed to handle my complaint or escalate within the bank if required. They can't even provide reference numbers for the calls made to their call centre.
- Absa has failed to close the credit card on my name and reverse all fees charged.
