SAA Complaint: terrible service & plane quality

We recently flew with SAA and the return flight was not fit for purpose.
The reasons are:

1) During dinner I started mildly choking. My wife flagged a stewardess and indicated it was urgent that we needed water, pointing at me choking and the stewardess indicated she would bring some. 10 minutes later the stewardess had still not returned. It was only quick thinking by other passengers as they sourced a bottle from another seat. The stewardess was aware that this wasn't a regular 'water requirement' and yet did nothing to help an urgent situation.

2) The plane was far older that the plane we left on and included no personal TV's despite this being a advertised feature (our outward flight included one). We were left on a flight with TV's in the front playing a movie (Seventeen) which was over 3 years old. This is unnacceptable for the cost we paid, as well as it being a advertised feature.

3) After dinner all of the passengers in our area waited at least 50-60 minutes for our trays to be removed.

4) The windows alongside me was actually broken, it was pushed in towards the external window of the plane. This clearly wasn't serviced.

I would appreciate a response.
