Overcharge on account for handset never received

Mr EF Ellerbeck a 75 year old pensioner has paid for a contract since around March 2012, which includes a Samsung 10.1 tablet which he never asked for and never received. At that time he went to Autopage branch in Vaalpark to renew his contract and declined any upgrade or renewal of handset as he was comfortable with his existing "old "phone. He was totally unaware that a Tablet was included in the renewed contract and he never received the tablet.
After moving to Tableview in December 2012 I noted his account charge seemed excessive and his phone needed to be replaced. At Autopage Tableview we were assisted by Mark who discovered this situation on the contract details and noted there was a lack of information which prevented the identification of the Autopage consultant who processed the upgrade in Vereeniging. Mark tried to resolve the issue but after many weeks admitted defeat. Eunice from the Vaalpark branch has also been assisting the Table View branch with this, and from what I understand has been very helpful. Although we are receiving feedback from the stores, we are not seeing any resolution. This contract must be cancelled and a full refund made.
