Soft locked because of no debit orde

My company phone was transferred to my name on the 24th of January with the debit order date being the 27th of every month. We also took out an additional contract for my wife with the same debit order date.

On 27 January, only my wife's previous contract amount was taken from my account and last night at around 23:00 we both received sms's that our phones are now soft locked.

An enquiry this morning indicated that we've exceeded our credit limit of R1000 and I'll have to pay money in to rectify the situation. After paying the money in, the situation is still not rectified.

How could they
1) Fail to debit the correct amount?
2) Cut my lines without consulting me about it?

I would have expected them to follow the following procedure:
1) Phone me and tell me that they've made a mistake in not debiting the correct amount and if I would be so kind as to make the required payment, or ask permission to take the full amount from my account.
2) Just keep quiet about their mistake and increase my credit limit and take both month's payments on this month's 27.
