Money from my bank account!

Money from my bank account, for a cell phone account thats not mine, and going for the third year! I've been to the Centurion branch since the beginning of last year to stop the debit order, I still have to go to the bank, every month to reverse the payment. Every time I go to the branch, they tell me it's a account and they are investigating it, but it still goes of on my account?!?
They expected a sworn affidavit from me, I did it, took it to the branch and they told me it will be sorted out in a week, after it went of again I went to them and ask for an explanation, they were not happy with the affidavit, due to the fact that it was done by a commissioner of oath, but not at a police station, in the first place, why did I have to do that?
Went to the police station, got it, again, took it back, but still no joy.
I received a ref nr (101478164) but thats it!
