Hounding for payment of purchases I did not make

My account was paid in full in Dec 2012 & I made no purchases thereafter. I received an account saying that I owed money in Feb 2013 & when going to the Beacon Bay branch to query it, I realised I had someone else's card & therefore they had my card. 2 purchases were made by this person obviously before noticing they had the wrong card. I immediately went to the police station and got an affidavit stating the above & took it to the BBay branch & was told it would be sorted out, this I think was 18 Feb. In the following 4 weeks I received numerous calls regarding my unpaid account & I explained the above to every caller. I then received a call from the BBay branch to say that my affidavit was lost & I needed to bring in another one. Last week I took another affidavit into BBay Edgars, filled out the forms and was told it would be sorted out. Today I got another phone call about the outstanding account and for the umteenth time explained my story. This evening I received a letter from Lee-Ann Daniels (Collections Manager) to say that my payment history is on record with the credit bureaux. This makes me HIGHLY annoyed as I make sure my credit record is perfect! FIX IT IMMEDIATELY!!!
