Billing for second handset after being returned

I did an upgrade to an iPhone 4S in August 2012 which was returned and confirmed by the Rewards Company (an agent of Vodacom). I have been and continue to be charged for this returned handset for over a year and several attempts at logging calls to rectify this error has resulted in nothing at all.

I still get my monthly bill charged for two handset installments for the same number. Even though Vodacom has acknowledged the issue and the error, they have (after over a year) still not resolved the issue and only give me lip-service whenever I call them about the problem.

Even after threatening to migrate two of my numbers (due for upgrade), post this complaint and to contact their CEO, they still have not taken any positive action to resolve this issue.

Clearly customer service nor customer retention is very high on their agenda and they don't give a as long as the error is in their favour. I am now out of pocket for over R2, 500 and have no idea when (or whether at all I will see this money).
