Cancellation not done

Twice I have faxed you the cancellation form, yet the account is still active, I want this canceled today, Using your service is worse than having a 250mb capped account (having to watch how much we use and your AUP business etc) I cant have m web as an ISP under these conditions, cancel with immediate effect, I am now with a proper ISP that offers a proper uncapped service, User name is below
