Need my money to be refunded immediately

In August my 8ta contract expired and a lady called me in September to upgrade it and I advised her that I had lost my cellphone and my sim card thus I would like to cancel the contract. She said I should email [Email Removed] which was done on the 11 September 2013. In October my account was once again debited. I again sent the email on 1 November advising that I had asked for a cancellation. On the 11th November I sent an email to [Email Removed] requesting the cancellation and they responded by advising that I had to complete a cancellation form which I did and forwarded to retainme again. But this was never actioned.
During the September to November I received calls to say I qualify for an upgrade and everytime I had advised them that I was no longer interested. In October had a call from SellDirect and the handset was delivered to me but after sending them an email requesting for the handset to be picked up this was on the 25th October. The package was collected and I thought the contract is cancelled. R197.00 was taken December which made me so upset. I have lost R394 as my account is still active. I sent numerous emails to cancel but to no avail
