Not giving credit and over charging

I have requested that you cancel the 2Gig recurring data bundle on my account on 21 August 2013 (check ref nr M130821219), but still you have kept it active for another month (September where you only cancelled it at the end of the month) and you have also invoiced me for this 2Gig on 1 September 2013, even though I have cancelled it in the previous month. Since then I am struggling for Autopage to give me credit for this 2Gig bundle which I have cancelled. Ref nr with your call center where I queried this: M13091745 and A1309175332. I phoned again on 1 November 2013 where it was apparently escalated, but nothing happened now after 5 weeks again. Seriously? What should I do to get the credit note??? You constantly give bad service!!!
