BAD SERVICE and failure to inform client

I qualified for and took an upgrade with MTN in October 2012 where I was offered insurance with the phone, they failed to mention that the insurance would be supplied by an external company and all I was told was to complete the consent form and that the insurance would be a separate debit order.
I then changed my banking details with MTn and they failed to provide these details to the insurer or even have the decency to text or call with regard to premiums not being received for the insurance.
My cellphone has now stopped working and only today did I find out I am not insured with them and that the premiums are outstanding.
As per the CPA they needed to inform me of this which they failed to do and now I am stuck with a phone that does not work and cannot be repaired or replaced.
I am extremely upset by this lack of good service and believe that they have in a way breached an agreement made with them due to lack of communication and no one seeing the necessity to inform me of premiums not being received. I am contemplating cancellation of my contract if this matter cannot be resolved as this is not my fault and I will not be inconvenienced in this manner when I pay them monthly
