Waiting a month for response and action

On 5 November 2013 I lodged a service request for the replacement of a phone I had just received on an upgrade. This was well within the 14 days refund / exchange period as promised by Vodacom when I took out the new contract. Today is the 5th of December and I have still not had the wrong phone collected and the correct one delivered.
I have spent several hours over the past 4 weeks holding on for various people in the Upgrades department to arrange collection and delivery. No one has taken responsibility, I simply get passed from person to person and between departments.
I dealt with a supervisor in the Upgrades department, Andries Mantokeng, who over the course of 2 weeks constantly promised to resolve the problem but then didn't. He then told me I had to speak to Kagiso Matlala in the Online Upgrade department. she said she would "send an email" to Sales Support and call me back, but of course she didn't and still no action to collect and deliver the right phone.
The new contract has kicked in (on 30 Nov), and as a result I now have no data on my current (old) phone. I am at my wits end. I am angry, frustrated and feel the lack of service on this matter is unacceptable.
