Debit orde activated without consent/ service

My lines have been disconnected from the Vodacom network, i sent through a request for one line to be connected, followed up to confirm request was advised that re-activation wont be processed as I need to re-activate both my 2 numbers. I told them I will send through the other sim card, on the 21/11 R388.?? was debited from my account. called in on the 22/11 to enquire about the debit order, they couldnt confirm why I was debited and told me someone will call me back. then A RUDE INCOMPETENT somebody called me back and told me shes the 1 who activated the debit order, I asked her why?? cause I havent sent through the 2nd sim card and my other line is still deactivated, she said she received the same request as the first one but thought it was a 2nd sim card number. she even blamed me for her STUPIDITY accusing me of send the same sim card twice.. Instead of her apologising for STUPID mistake she went on and gave me an ATTITUDE from... She is an ABSOLUTE DISGRACE to your department, She will call me back and apologize for her attitude she must learn to listen and value customers!!! I dont want the team leader/ supervisor I want the very same person TO CONTACT ME!!!
