Mnt service provider debited on my bank account

They claimed that i ordered a cellphone over the phone which i never did and i stopped debits but then it accumulated while they were not getting the money. All that started in 2011 Dec when i went to report they said my identity document was used but i never lost it, now i have a bad name credit wise so please assist me in getting to the bottom of this because the police are sending me back to get the address for where my money was taken from and that is so impossible to find after this period of time
