Infinite instament without records

I had a contract with mtn for 24 months in July 2008 which was paid through debits monthly. Suddenly debits were stopped but kept paying inside bank hence keep asking mtn to debit the account but never debited again. I then some months couldn't pay as I had to go to the bank and working far from towns in coal mines. Eventually my services were terminated and vvm started collecting R500 monthly from my bank. I then asked for details of my account and term to finish paying it since 2011 till now I didn't receive anything from vvm. May I please be supplied with details of this account with ref.
