10 months and 4 cancellation req to cancel contrac

In Nov 2012 our cell phone number was due to expire. we decided to cancel to contract as we have work cell phone contracts. We visited the Clearwater office and completed a form to cancel in January 2013, and was advised that it will be cancelled after 30 days. When we receievd a statement in March we realised that the account was still not cancelled, and went into the Clearwater office again. The consultant advised that they can see the cancellation was logged but it was never actioned and we completed another form and was told it would take another 30 days to cancel. In June we realised that this was still not cancelled and visited the Clearwater office again, the consultant made us sign a cancellation form and assured us it will be cancelled now after 30 days... again... Recently we checked the account and realised that the contract was still being debited to our account and still not cancelled, BIS billed monthly and not used since March. We saw a consultant who asked us why we only noticed it now? We again completed forms and requested a refund since January due to the bad service. We did not receive any feedback so logged a call at the call centre last week, but still no feed
