Failure to provide feedback

I have been a customer for more than four years now and have never missed a payment. I have been contacted for a renewal on my phone, however each time I am contacted I am promised feedback however no one ever contacts me back and informs me when if my renewal has been accepted.

I asked the person who contacted me last time to inform me if my renewal was not accepted, because I would then like to cancel my services. The guy never called me back and when the end of the month came I was charged again. Over and above that when I pulled my latest statement I can see that the billing on it is incorrect. Please can I get a final decision from you guys if you wish to keep me as a customer, if not, fix my billing and let me out.
