Can not report a fault

To Whom It May Concern.
I am unable to log a fault on our ADSL line because there is still an outstanding fault logged since 28 March 2013. (Reference: 521CNK280313)
the previous fault was fixed over a month ago, but since it is still open on the system I am unable to log the new fault.
Please can you log a new fault (ADSL line not working) and clear the old fault.
My contact details are the same as the previous fault (521CNK280313).
