Failed Attempts to terminate contract

My contract came to an end at the end of January 2013 and ever since I have been trying to end my contract. I have given a months notice, two and three months notice at different times as the notice period changes every time I call to terminate my contract or go into the store. Every month I am assured that I will no longer be paying for the contract as it has NOW been cancelled yet the 1st of every month the money goes off my account. I feel this is unacceptable as I have been trying to get out of this contract for the past five months. I do not owe any money and my contract has run its full term. Each time I call I receive a new refereance number yet when the next month comes and the money has gone off my account for some reason they can not trace the referance number and apparently there is no indication that I have cancelled my contract. It is day light!!
