Wrong debit orders and a breach of contract

Planet fitness told me that I'll have to resign a contract that head office needs to confirm the amount of R311 on an Edgars discounted price, however they included a 2X levies fee R415, which I'll have to pay in December, that they never told me about which I wouldn't have taken the contract in the first place if I knew about this fee. I went into the store to pay my fee of R311 for my membership fee which they told me i can do so that i don't get the R445 deducted from my account. my acc went into arrears again as they failed to stop their debit order as they promised end of Sept. 15th Oct PF tried to debit my account with R200 and since the account was in arrears it hopped. At the end of October they deduct a random amount of R159 form my account with no permission of any kind. The incorrect fee of 445 went of my account AGAIN on the 1rst of Nov, still nothing has been done about it. I complained to the manager once and he tried to phone me the next day, only once, and since I was at work and couldn't take calls. No action has been taken to rectify this contract issues. Nor any apologies from head office, FIX THIS NOW!!
