Medical aid refuse to pay

I went foa a vasectomy on the 13th March 2013. After giving all the ICD 10 codes and procedure codes, and telling them its goingbto be at montana netcare hospital, they gave me the authoration number and said everything is done. A week later they refused to pay the full amount, because now they say they only pay for it being done in docters rooms not in theater. My docter wrote them a letter motivating why this needs to be done in theatre. Today i recieved an email from montana hospital, stating that i have to pay the outstanding amount immediatly, medical aidcrefuse to pay. Why do i have a medical aid if they only pay what they want when they want. The makehondreds of million profit every year, but they cant do what you psy them to do?
