Unhelpful and wrong paperwork... Shocking!!!

I had an outstanding account with a Dr after having my baby as my medical aid decided it was not going to pay for everything, I was unemployed and did not expect this therefore I have had to try and pay it off as I could. However the last amount I really struggled and they handed me over (without even telling me) since then I have paid the full outstanding amount to the Dr, however this legal firm keeps contacting me telling me I need to pay their fees, I have instructed them to send me a statement as well as the document i signed acknowleding that i woud cover legal costs for outstand debt... i have since recieved the statement from the dr, which is not up to date and doesnt show my last payment to them as well as my patient details with all my personal info, but then I get a signed document that agrees to pay legal fees however it is not even my name or signature, and Khozi just keeps sending it to me and obviously not listening to the fact that I am not fahima peterson, the person who signed this! They also call on a pvt number and i have managed to get a number off the internet but it doesnt work... so i have no idea how i am supposed to resolve this...
