Issuing a debit order on my account

I have recently noticed a strange debit order going off my account with much searching I discovered that under instruction from Virgin Active MBD Attorneys issued a monthly debit order withour my knowledge or approval. I believe that this is and should be dealt with, I have called them numerous times and thy always tell me someone will call back todate no one has called and I want this debit order stopped. The bank cannot assist me as this has been happening since August 2012. My son passed away in September 2012 and therefore I was not in the emotional state to follow up on this problem. I would like to have my monies refunded immediatly.
