Refund for bank charges incurred

My debit order for September 2012 was returned insufficient funds by my bank (I was aware of this).. I called the call centre and requested that the resubmission for another debit order on 10/09/2012 be cancelled as I will still not have funds in my account until the 25th of September and I understand that I will not have cover during this period. The lady that I spoke to at the time said it is fine the resubmission will be cancelled.
To my surprise I received an sms notification on the 01/09/2012 that a debit order of R318.70 has been submitted by Outsurance and I could only query it on next working day; 03/09/2012. I called the call centre on 03/09/2012, spoke to Miguel and he told me that i needed to send in proof that the debit order was submitted so that the bank charge R138.00 of the returned debit order can be refunded to me. He requested that I get a bank stamped statement and send it to him [Email Removed] and the refund will be done.
But then another debit order of the same amount was submitted on the 17/09/2012 causing another returned bank change of R138.00. Shaun is the one that promised that the bank charges will be refunded on the 25/09/2012. I have all emails
