Service provided on 4/12/2013 and still unpaid

I provided a service to a Bankmed member on 4/12/2013 and received a letter dated, 23/12/2013, stating that this particular claim was received and will be processed with their next claims run for service providers.
On 16/01/2014, when I queried this claim which was still unpaid, I was informed that it was never received. I resubmitted the claim with their letter dated 23/12/2013.
Yesterday, 28/01/2014, I spoke to a call centre agent, Bradley, who informed me the claim will be paid on 31/01/2014. I wanted to speak to a supervisor and was informed by Bradley that the supervisor, Nolu, will phone me after I gave my contact details. I did not receive her call.
I would like this pathetic customer service investigated.
