How can I get my money back?

1/8/2013 Paid co-payment with my credit card for a procedure to be done that morning. the procedure was
Cancelled because the Doctor was taken ill.
2/8/2013. Went to the office at the Glynnwood and filled in a form for the refund. I was told that I would have it by
Monday.
6/8/2013. Phoned to speak to Lucas but he was unavailable. Left message for him to phone me. No response.
7/8/2013. Stopped at the office and spoke to Lucas. all in order. He then phoned about 30 mins later to say my
ID was needed.
10/8/2013. Left copy do ID at office.
12/8/2013. After numerous calls and no response, Lucas finally phoned at 3.30 to say all was in order and I could
Expect the refund within 2 days.
15/8/2013. No money yet. Went to see Lucas and it will be in my account by Friday!!!
18/8/2013. As yet, no refund.
HOW CAN I GET MY MONEY BACK???
