Poor Feedback and assistance

I received a call from a Debt Collector a month ago for an outstanding account for 2009, I told her I need to look into it 3 days later I received a notice from ITC telling me that this account was loaded on to my credit profile. I cannot get any help from the Dr's office or the Debt Collector to help me sort this account out. I didn't receive a call or post from the Dr's office telling me that any moneys are out standing on my account. According to the lady at the Dr's office they called me in 2011 and I said I was going to pay this account, I don't know who they phoned because the contact details they have is incorrect. After all the hassle they sent me the incorrect statement and it took days for the correct statement to be mailed to me.
