Billed twice and no refund after 10 days

I went through the drive through nearly 2 weeks ago, there was a problem with the credit card machine after I had entered my PIN and nothing printed out.
I was then told by the cashier that I needed to swipe again and if the money came off twice I must come in with my bank statement for a refund. I was in a hurry so didn't have time to argue and swiped again.
Of course the money came off twice and now I have repeatedly tried to get it reversed with no result.
How dare he tell me I need to bring my bank statement in for their mistake. I have already been to the branch twice to resolve this and now it is apparently being dealt with by head office.
The bottom line is I am short money that they have taken and it has not been refunded after nearly 2 weeks.
On this same occasion I was to by the person at the collection window. He told me there were no regular fillets only Zinger ones, and asked if that was OK. I said no, it was not OK, I would like what I ordered. When receiving my order I asked if it was a regular fillet and he assured me it was. Upon returning home I found that it was a Zinger burger.
How dare you treat your customers like this, please resolve this immediately!
