No card after a month

On 23/3/2013 the ATM swallowed my debit card. The staff was very helpful in issuing temp card, and promised to order a new card. After 2 weeks I phoned to enquire if the new card arrived, only to find out that it was not ordered. In the meantime one of my other accounts, a savings account, was changed from a savings to a access account, without me asking or authorising it. I now cannot transact from my savings account, without going into the bank first to activate it, and cannot access my internet banking, because of the tempory card. As of today I have still not received a card, a month later. Now I need to drive around in order to pay accounts. Dangerous! This is the second time in three years that this has happened. Cannot believe that we get such bad service from a bank, to which we pay bundles of money every month!!!
