Not adding beneficiary after they said I could

Me & my live in partner are working for the same company & thus have medical aid with Discovery through PSG (Their Broker). She fell pregnant & so we decided that instead of paying for 2 separate medical aids, she joins mine & I just pay. Now at the beginning of Jan I added her as my beneficiary, filled in the forms & sent them through my payroll department to forward them through as I could not do it myself (procedure?). Till today I have not received any form of Documents from Discovery for my new pan-upgrade via email, post or sms. I got an email from my payroll on the 15/02/2013 to confirm that would I still like to add her to my medical aid? Obviously since I completed a survey & filled in forms, so I email back saying YES. Now assuming she was added to my medical as she calls Discovery requesting an Authorization number for the Hospital she is told that not only will she not get the number but she is NOT LISTED UNDER MY MEDICAL IAD!? Now they are refusing to add her saying that because I have a higher plan it would be seen as an upgrade & she cannot be added which is not what their Broker PSG informed me of, so who is liable? And I have proof of this. So to cut a long story
