Vitality cancellation

I have a Discovery Medical Aid for more than 10 years now and I'm the principal member. A couple of years ago I added my husband as well and I have always managed my medical aid. I cancelled my Vitality option in Nov 2011 telephonically with the Discovery Call centre and was given a reference no which was forwarded to my payroll department. It was only in Nov 2012, that I discovered that the Vitality deductions had continued from my salary. I consulted with the payroll department who advised me that they did not cancel the deduction due to staff error & as a result the deductions were paid to Discovery and therefore a refund was not possible. I am very upset as Discovery was informed, payroll was informed and payroll acknowledged my email and yet the fault is all mine as I failed to inform them as I did not look at my payslip and advise all parties AGAIN. Discovery has now advised me that my payroll department had informed them in Dec 12 - a year later! My question to Discovery is what happened to my instruction made directly to them in Nov 11. Why had they failed to inform me that the cancellation was not done? This is unacceptable!!
